- Provides case management and referrals for essential and supportive services, for a single access point for clients needing home care services.
- Intake and Eligibility to determine clients who meet the criteria of the programs and needs for services.
- Culturally sensitive client assessment that involves clients, family, caregivers, and service providers. Includes client needs assessment and care plan.
- Provides a safe and effective equipment lending system for medical equipment and supplies, on a short-term basis for clients.
- Refers to Algonquins of Pikwakanagan Home Care for homecare services.
- Refers to Home & Community Care Supportive Services Champlain (HCCSSC): home nursing, physiotherapy, occupational therapy, etc.
- Maintains inventory for PPE supplies.

Chronic Disease Management Nurse
- Provides the First Nation community with culturally safe health education on disease prevention, healthy lifestyles and preventative screenings through educational workshops and events in collaboration with other health care professionals
- Self-referral or physician referral appointments for support in navigating and managing chronic conditions
- Provides assistance and education for continuous glucose monitoring systems (Dexcom’s and Freestyle Libre’s)
- Delivers group and 1:1 fitness programs that are Heart Wise Certified through The University of Ottawa Heart Institute (chair exercises, nordic walking, group fitness, walking programs)
- Collaborates with surrounding hospitals and healthcare professionals to coordinate and integrate services within the community, enhancing accessibility to care. These services include nephrology consultations, telemedicine appointments with specialists, diabetic retinopathy eye screenings, Pembroke Regional Hospital Diabetic Outreach Team, and many more additional specialized healthcare services.
