The Algonquins of Pikwàkanagàn provides a holistic home health care program that is reflective if its’ health and social needs, that is comprehensive, accessible, effective and equitable to all its’ residents.

The Home & Community Care worker provides culturally sensitive assessments, referrals and case management via a single access point for clients needing home and community care. It is designed to maintain the elderly, individuals with disabilities and/or chronic diseases in a manner that promotes independence and is supportive to family caregivers. It can improve the quality of life and decrease the number of complications, injuries and acute illness experienced by individuals with disabilities and chronic disease for all age groups. Also, to receive the care they need in their home and in their community.

Client Assessment:
This part of the Home & Community Care program involves talking to you and your family, talking with your family doctor and others, talking to people who have cared for you, reviewing your health history, and reviewing your current health status.

Case Management:
After your assessment,a case management/managed care process incorporates a case management, referrals and service linkages to existing health and social services. Referrals to Health and Social Services include Home Nursing Services through CCAC, Provision of Home Care, Home Support, Personal Care Services, Physiotherapy, Occupational Therapy, Nutritionist and In-Home Respite Care.

Equipment Lending System:
Access to specialized medical equipment and supplies.

Patient Advocacy:

Coordinate client transportation as requested.
